HUMAN SERVICES TRANSPORTATION DELIVERY
Human Services Transportation Delivery
- provides transportation services to the eligible clients in the Buffalo Trace Area.

Recipients who need to schedule transportation services should contact us during our normal office hours of Monday through Friday from 8:00am – 4:30pm and Saturday from 8:00am – 1:00pm local time. All trips not considered “urgent care” must be scheduled 72 hours in advance   After hours paging is available, including Sundays and State Holidays, for immediate or “urgent care” transportation.

Licking Valley Community Action Program currently operates the Transportation Program for the Buffalo Trace area. Licking Valley Community Program covers the Buffalo Trace area which includes counties of Bracken, Fleming, Lewis, Mason, and Robertson. Currently, Licking Valley Community Action Program operates the Public Transit Program which includes regular public transportation, Title III-B Elderly Transportation, Non-Emergency Medical Transportation, and Head Start. The 5310 vehicles are utilized by all participants except Head Start.

PUBLIC TRANSPORTATION

Anyone that does not have a Medicaid card or has a Medicaid card but does not meet other eligibility requirements for transportation through the HSTD Brokerage can call and schedule Public Transportation.

As a FTA Grant recipient of 5311 and a transportation provider, Licking Valley CAP provides transit services in a diverse manner to the residents of the Five County Buffalo Trace Region. They provide local Public and demand response medical transportation services.

Residents who are interested can call 1 (800) 327-5196 to schedule transportation. Public trips need to be scheduled 72 hours in advance. The reason for the early notice is so a coordinated schedule can be prepared and dispatched to the drivers.

There is a fare for all public transportation. The rates are different for in county transportation which is “transportation that originates and ends in the same county” than out of county transportation which is “transportation that originates in one county and ends in another”.

Public Transportation services are provided 5 days weekly Monday through Friday from 6 am to 6 pm. Exact change only, the drivers do not carry change.

 

Passenger Rules and Regulations

·            Must be identified by program eligibility

·          Shall have proper identification

·          Is not under the influence of alcohol or illegal drugs

·          Is ready 15 minutes before the scheduled pickup

·          Shall abide by signs in the vehicle (e.g. no smoking, no food, etc.)

·          Shall wear a seat belt and children shall abide by child restraint seat belt requirements

·          Shall not hold children in laps

·          Is responsible for personal items

·          Shall call 72 hours in advance for transport, if possible

·          Shall cancel the trip as soon as possible but no less than 24 hours in advance

·          Shall give all pertinent information needed to the transportation provider such as attendant requirements, special needs (e.g. wheelchair service)
